If you are at the Examination Room safe in Resident Evil Requiem and want the answer immediately, use Right 30, Left 10, Right 50 on the normal difficulty settings. On Insanity, the combination changes to Left 50, Right 30, Left 90. Treat the direction as part of the combination: stopping on the correct three numbers with a wrong turn direction will not count as the same input.
How to enter the combination cleanly
For normal difficulties, rotate right until the dial reaches 30, reverse to the left and stop at 10, then reverse again and go right to 50 before confirming. On Insanity, use left 50, right 30, left 90. If you overshoot or turn the wrong way, start the sequence again rather than trying to repair the input halfway through. The safe treats each direction change as part of the code, which is why players sometimes think the numbers are wrong when the real problem is the dial movement.
- Normal starts by turning right.
- Insanity starts by turning left.
- Only confirm after reaching the third number.
Where is the Examination Room safe?
The safe is inside the Examination Room at Rhodes Hill Care Center during Grace's sections. After story progression opens the East Wing route, you can reach the room and find the safe in a cabinet or recess toward the corner. Evil Resource places it in the southeast corner of the Examination Room, while PowerPyx describes it inside the cabinet in the room. If the East Wing is still inaccessible, the code is not your blocker; you simply have not reached the required story access yet.
Where is the clue file?
The Examination Room Safe Code file is not sitting next to the safe. You find it later in the Care Center's second-floor Locker Room, on a collapsed locker along the south side of the main locker area. Evil Resource lists that exact location for Standard and Insanity, and PowerPyx also tracks the file later in the Care Center route. Knowing the combination lets you open the safe before reading the note, so a repeat run does not need to wait for the document.
What is the reward?
The safe contains three Antique Coins. That matters because Grace can spend Antique Coins in the Parlor for upgrades, making this more than a completionist pickup. The Examination Room safe and the Bar & Lounge safe are especially useful early sources of coins. If you are planning a specific upgrade path, grabbing these three while you are already in the room is more efficient than discovering later that you are short on currency.
If the code fails, check these two things
First, verify your difficulty. Normal uses Right 30, Left 10, Right 50; Insanity uses Left 50, Right 30, Left 90. Second, verify the rotation directions. The combination is not random, and current guides from Evil Resource, PowerPyx, and GamesRadar agree on both versions. If the dial resets, restart from the first number with the correct direction rather than assuming the safe has changed for your save.
Does this safe matter for trophies or achievements?
Requiem has multiple code-locked safes, and opening them is tied to the No Safe is Safe trophy or achievement path. PowerPyx notes that the numeric safes are encountered in Grace's gameplay sections and that their combinations are fixed for the relevant difficulty. Because the game does not offer a simple chapter-select cleanup route, manual saves around major areas are useful if you are collecting everything. Opening this safe when you first reach it avoids another reason to backtrack.
Before you move on
- Confirm your difficulty.
- Normal: Right 30 → Left 10 → Right 50.
- Insanity: Left 50 → Right 30 → Left 90.
- Take the 3 Antique Coins.
- If you collect files, pick up the Examination Room Safe Code note in the second-floor Locker Room.
